Cluster Headache - A Migraine-Like Disorder Marked By Attacks Of Unilateral Intense Pain Over The Eye And Forehead, With Flushing And Watering Of The Eyes And Nose; Attacks Last About An Hour And Occur In Clusters.

Cough Headache - Severe, Stabbing, Bilateral Head Pain Brought On By Coughing Or Some Other Valsalva Maneuver.

Exertional Headache - One Occurring After Exercise.

Pathological Point Of View -
A Headache Or Cephalalgia Is Pain Anywhere In The Region Of The Head Or Neck. It Can Be A Symptom Of A Number Of Different Conditions Of The Head And Neck.

Pathophysiology :-
The Brain Itself Is Not Sensitive To Pain, Because It Lacks Pain Receptors, Also Called Nociceptors. However, Several Areas Of The Head And Neck Do Have Nociceptors, And Can Thus Sense Pain. These Include The Extracranial Arteries, Large Veins, Venous Sinuses, Cranial And Spinal Nerves, Head And Neck Muscles, The Meninges, Parts Of The Brainstem, Eyes, Ears, Teeth And Lining Of The Mouth.

  • Most Headaches In General Practice Are Ordinary Tension Headaches And Need Just An Analgesic And May Be A Tranquilliser. 
  • Advice Patient To Take Rest In A Dark, Quiet Room. Massage And Press The Scalp Apply Counter Irritants Like Vicks / Amrutanjan To Forehead.
  • In Every Patient Of Headache, You Must Test The Following Signs, As Routine:-
  • Check B.P. - Control Hypertension If Present. 
  • Check Sinus Tenderness Over Maxillary & Frontal Sinuses, And Look For Any Focus Of Infection In Teeth, Tonsils & Ears. 
  • Check Tenderness On Nape Of Neck Or Pain On Neck Movements. 
  • If Associated Fever, Check For Neck Stiffness. 
  • If Eye Is Painful Or Red, Palpate The Eye After Closing The Eyelid. If It Is Firm To Feel, Refer To Ophthalmologist For Glaucoma.


  • If Headache Is Recurrent & Chronic :-
  •  Check Eyesight First. 
  •  Check BP, Sinus Tenderness & Spondilitis As Above. 
  •  If It Has Started Following Head Injury, Refer For X-ray Skull & C.T. Scan. 
  •  If Patient Is Diabetic, Is He Getting Hypoglycaemia ? 
  •  If Headache Is Severe & Progressive, Refer For C.T. Scan. 
  •  If There Is Any Neurological Sign As Weakness Of Cranial Nerves Or Limbs, Convulsions Etc. Then Refer Immediately For C.T. Scan & Neurologist's Opinion.
Some Points To Be Kept In Our Mind During Handling The Case Of Headache :- 

  • Headache With Fever = Meningitis. First Test For Neck Stiffness. Malarial Fever Also Gives Headache.
  • Tension Headache Is The Commonest Headache. It Is More Towards Evening & Patient Feels Better On Pressing Over The Scalp.
  • A Young Patient Or Child Complaining Of Repeated Headache Will Usually Have Refractive Error. First Get Eyesight Checked. Particularly Cylindrical Numbers Even When Very Minor, Give Rise To Headaches.

  • Headache Due To Alcohol Hangover :-
  • Take 2 Glasses Of Water - Plenty Of Water And Fluid Intake. 
  • Take Simple Analgesic. 
  • Take Fruit Juice, Or Tea Or Coffee With Plenty Sugar ( To Counter Hypoglycaemia ) 
  • Have A Full Meal.
Prevention Of Hangover :- 


  • Avoid Excess Alcohol. Stay Within Your Limits. Avoid Cocktails. 
  • Do Not Drink On Empty Stomach. Eat Something Along. 
  • Take A Glass Of Water After Each Drink. 
  • Do Not Drink Too Fast.

Migraine :-
Treatment Of Acute Attack :-

  • Lie Down In A Dark & Quiet Room. 
  • Give Ice Bag Over The Head. 
  • Strong Analgesics. 
  • NSAIDs.
 Some Points To Be Kept In Our Mind

  • Migraine Is Characterised By One Sided Severe Headache Lasting For Several Hours, With Intervals Of Complete Freedom. It Is Associated With Vomiting And Visual Disturbances.
  • Vasograin Or Migril Should Be Taken As Soon As Prodromal Symptoms Appear, To Abort The Attack.
  • Maximum Dose Of Ergotamine Tartarate Should Never Be Exceeded. i.e. 5 Tabs In A Day And 10 Tabs In A Week.



If Migraine Attacks Are Frequent I.E. More Than 2 Per Month :-

  • Avoid Factors Which Precipitate The Attack. E.g. Sleeping Late, Tension, Starvation, Certain Foods Like Chocolate, Cheese, Alcohol. 
  •  If Control Is Not Adequate, Add Anti-Depressant.
